about6The Medical Social Worker meets with the patient and family/caregiver and helps them to make adjustments to the impact of illness.Services are initiated on an individual basis when there is a concern for the patient's economic, environmental or personal safety conditions. We also help the patient to access financial and community resources.

 

 

The services of Medical Social Workers include but are not limited to:

  • Assess social and emotional factors related to patient's illness.
  • Provide information about community resources available for the patient.
  • Assess patient's psychological needs and communicate the status with the home health personnels.
  • Assess relationship of patient's medical and nursing requirements to the individual's home situation.
  • Assist in helping the patient reach independence
